Abstract     Angiotensin-converting enzyme (ACE) inhibitors slow down the progression of renal failure irrespective of blood pressure. This was confirmed in patients with and without diabetes. The favorable effect of the renin-angiotensin-aldosterone system block was also proven by the application of angiotensin II receptor antagonists. The best therapeutic effect was described in patients with severe proteinuria. Both drug groups are about equally effective although angiotensin II receptor antagonists have fewer side effects. A combined treatment with ACE inhibitors and angiotensin II receptor antagonists has also been described in renal diseases, however, no final standpoint has been taken regarding this method of treatment.
