Abstract     The case concerns an 79 years man who collapsed while working in the field. Upon the arrival of the Emergency rescue team to the site of the incident, the patient was unconscious and obviously in shock. I performed detail clinical exam, but I still couldn't be certain about the cause of shock. Although the causes of the shock may vary, the therapy remains more or less the same, regardless of the cause - or it can be significantly different as illustrated in this case. At the end I decided that anaphylaxia is the most probable cause of shock, but the decision to administer epinephrine was especially diffcult. Considering the patient's age, it could have rather quickly triggered the hypoxia of the myocardium, if the latter of course, was not itself the cause of the patient's condition. The treatment of anaphylaxia was, despite my doubts, the right choice. After he had spent a few days in hospital, the patient recovered and went home.
