Abstrakt     Long term survival rate in pre hospital environment among other factors depends on Emergency Medical Services response times. Response times among other factors depends on Emergency Medical Services organization. Late notification, low bystanders first aid percent, long response times and large cover area with central ambulance positioning have largest influence on long term survival rate in pre hospital environment in Ljubljana . Findings from recent studies in Ljubljana and surrounding area showed that mean response are between 10 and 11 minutes with 6,6% (5,6%) long term survival rate. To increase long term survival rate in pre hospital environment authors recommended: implementation of wide spread basic life support educating program for lay persons, implementation of first Emergency Medical Services component, actions for shortening response times. To study and realize recommended actions we analyzed 1165 (11% of all interventions) the most difficult interventions in pre hospital environment in Ljubljana and surrounding area. Based on analyze results we recommend several optimization measures: centralization and optimization of Emergency Medical Services dispatching based on computer dispatch technology, implementation of passive and active System Status Management, implementation of Early Defibrilation Program. In the case of 100% realization of optimization measures we can expect that response time will decrease for 50% and long term survival rate will increase on proper comparable level.
