Abstrakt		In organ transplantation, beside tissue compatibility between patient and donor it is very important to find out whether the patient has HLA antibodies, which might cause an acute rejection of a graft. We screen sera of renal patients on the waiting list for kidney transplantation four times per year. We determine panel reactive antibodies (PRA) and the specificity of HLA antibodies and forward the data to the Eurotransplant database. With the specification of "unacceptable" HLA mismatches we make the choice of recipient easier and safer. Before transplantation we perform cross-matching between the donor's lymphocytes and the recipient's sera. If the test appears positive the kidney is mainly transplanted to another patient from the same centre with suitable histocompatibility matching to prevent long ischemic preservation.
Izvleček		Pri presajanju organov je poleg ujemanja v antigenih HLA med prejemnikom in dajalcem izrednega pomena ugotavljanje morebitnih protiteles HLA v serumu bolnika, ki bi lahko povzročila akutno zavrnitev presadka. Pri bolnikih, ki čakajo na presaditev ledvice, določamo v presejalnih testih HLA štirikrat letno odstotek (% PRA) in specifičnost protiteles HLA ter te podatke redno vnašamo v podatkovno bazo Eurotransplanta, kajti s pravočasnim vnosom "nesprejemljivih" oz. "prepovedanih" neskladij v sistemu HLA preprečimo neustrezno izbiro možnega prejemnika. Pred presaditvijo ledvice obvezno naredimo še navzkrižni preizkus (XM) med dajalčevimi celicami in serumom možnega prejemnika, in če je ta pozitiven, se zaradi časovne omejitve ter velikih razdalj večinoma presadi ledvico drugemu bolniku iz istega centra, ki ima zadovoljivo skladnost in negativni XM.
