Abstract     Intramedullary fixation of trochanteric fractures is biomechanically one of the best methods and offers early reahbailitation and weight-bearing. It has some important advantages in comparison to other extramedullary fixation. The operative procedure is easier and more rapid, there is less blood loss, the bone healing is better and more rapid, the number of local and systemic complications is reduced. The intramedullary fixation is successfull only if all instructions and criteria for usage are taken into consideration. There are three commonly used intramedullary methods for the fixation of trochanteric fractures: classic intramedullary hip screw, long classic intramedullary hip screw and reconstructive intramedullary nail.
