Abstrakt     Introduction: With evaluation of late sequelae in long term childhood cancer survivors we recognize risk factors for their occurence, can define groups of survivors with high or low risk and consequently plan treatment accordingly to minimize late sequelae. After treatment of nephroblastoma, renal function can be imaired as a consequence of nephrectomy, irradiation of the remaining kidney and/or chemotherapy. At present, nephrectomy is mandatory and in last decades there is a tendency to replace irradiation treatment with chemotherapy. While the effects of irradiation are rather well known, the late effects after chemotherapy alone and after combined chemotherapy and radiotherapy are not. There are only few studies of renal function in long time survivors after treatment for nephroblastoma, the results of which are not all in agreement. Aim: To evaluate late effects of nephroblastoma treatment on kidney size and function. To analyze the influence of different risk factors like the age at treatment, mode of treatment (irradiation, chemotherapy) and follow-up period on the occurence of late effects. Finally, to define the most suitable method for detection of late effects on kidney function. Patients: Twenty-two long time survivors, 2 months - 16 years old at diagnosis, treated for nephroblastoma between 1969 - 1987 at the Pediatric clinic and/or Institute of oncology in Ljubljana were evaluated. The time of follow-up was 12,2 - 29,7 years (mean 24,4 years). At the time of treatment all had nephrectomy, 21 received chemotherapy, of which 3 received Avtinomycin (AMD) only, 13 AMD and Vincristin (VCR), 3 AMD, VCR and Adriamycin (ADR), 1 VCR, Adria, DTIC and Endoxane and 1 AMD, VCR, DTIC, Adriamycin and Endoxane. Six patients had no radiation treatment, 8 had tumor bed irradiation, 6 had whole abdominal irradiation and 2 had lung irradiation. (Abstract truncated at 2000 characters).
