Abstract     In this review we are going to describe the primary brain lesions produced by acceleration head trauma. This event is representing the start of the travmatic brain disease. Evolution and course of the travmatic brain disease will be described together with the factors affecting the course. The standard and new promising methods of evaluation of the brain affection in the course of the travmatic brain disease. Also their power to predict the outcome of the travmatic brain affection will be discussed. The late sequeli of the disease will be described. We are stressing that motor affection and affection of sensory functions is not the heavy burden of these patients. The affection of the intellectual and cognitive functions is more problematic making the family reinte- gration, professional and social reintegration quite difficult especially when the social environment is too demanding.
