Abstract     Although visual impairment is infrequently described in Alzheimer's disease (AD), this issue is becoming more relevant for the clinician as it is becoming clear that visual impairment occurs early in the development of AD, but remains often unrecognised, and because a subgroup of patients with an atypical form of the disease has been described whose main characteristic is a prominent visual impairment. In the article a clinical picture of the disease is described emphasising attentional deficit and oculomotor disturbances. The main methods for establishing and for the follow-up of visual impairments are listed (clinical examination, neuropsychology, structural and functional imaging, electrophysiological techniques, particularly visual evoked potentials, electroretinography and late endogenous potentials). Changes in peripheral organs are postulated (pupil, retina, optic nerve), but the main pathogenetic mechanism for the visual impairment in Alzheimer's disease is believed to be dysfunction of the hierarchically highest association cortices.
