Abstract     The aim of this study was to estimate wheather the serum lactate is real predictor of lactic acidosis. Incidence of lactic acidosis caused with metformin is present at 0.01 to 0.08 cases (average 0.03) per 1000 patients per year. It occurs in most patients because one or more contraindications were overlooked. In some countries there is a suggestetion to measure serum lactate as indicator of lactic acidosis in patients treated with metformin. Study was performed on 30 patients with type 2 diabetes mellitus who were newly diagnosed and without any others serious illnes. It was an open-label study. Patients received metformin (2x500mg/day) for five days. Patients visited us four times; in the day without therapy, in first, third and last day of therapy. We analyzed serum lactate at every visit in time 0, 60, 120 and 240 minute after per oral application of metformin. Patients were resting all time during the test day and they were pleased to avoid physical activity during the study. All anaerobic condition during the blood sampling were excluded. Variations of lactate concentrations in most patients during the same day in the same patient were very high (from 1.13.8 mmol/I). The majority of the results were in the normal range. There were no differences between the day without therapy and in days with metformin therapy. The most of the patients (76.7%) had at nearly all control levels normal concentrations of lactate and some others (23.3%) had almost always elevated values of lactate. The measurement of lactate in serum can not be used as a valid parameter as predictor for lactic acidosis in patients with metformin therapy.
