Abstrakt     The diagnosis of heart failure in clinical practice on the basis of symptoms and signs is rather difficult, because they are nonspecific. No clinical feature orphysical examination finding reliably distinguishes patients with heart failure who have intact left ventricular systolic function from those with heart failure caused by left ventricular systolic dysfunction. The diagnosis of overt heart failure and asymptomatic left ventricular systolic dysfunction depends on the measurement of LUEF by echocardiography. The use of LUEF measurements to diagnose heart failure has inherent problems. The LUEF varies in normal population. Besides, it depends on preload and afterload. The problem is even greater in heart failure and ventricular dysfunction due to diastolic dysfunction, because there is no universally accepted criteria for diagnosis of diastolic dysfunction. In both cases, determination of natriuretic peptides may be promising.
