Abstrakt | Invasive investigation is generally not required to establish the presence of chronic heart failure but may be important in elucidating the cause or to obtain prognostic information. Three diagnostic tools may be helpful in different situations: coronary angiography, haemodynamic monitoring and endomyocardial biopsy. Coronary angiography should be considered in patients with acute or acutely decompensated chronic heart failure and in the presence of severe heart failure not responding to initial treatment, in patients with angina pectoris or any other any other evidence of myocardial ischaemia. Monitoring of haemodynamic parameters by means of Swan-Ganz catheter is useful to assess the diagnosis and exclude correctable causes of heart failure. It is also useful in directing treatment of patients with chronic congestive heart failure. Endomyocardial biopsy may be useful in selected patients with unexplained heart failure. Because of associated high procedural morbidity and mortality, patients with severe ventricular dysfunction may not be considered viable candidates for surgical intervention. Often, these patients have undergone previous surgical revascularization, as well, further complicating the issue of repeat revascularization.
