Abstrakt | The absence of the missing lower limb is a major but a single impairment after amputation. Although the impairment can be appropriately treated with the application of a prosthesis, other impairments and disabilities appear after some time. With limb length inequality, which is frequent with prostheses, gait and posture symmetry are disrupted. Consequently, lowback pain and/orsciatic pain are common among lower-limb amputees. Due to the changed gait pattern in the amputees, forces to and loading of the knee joint on the non-amputated side are higher than on the amputated side as well as higher than in non-amputees. It is not surprising then that osteoarthritis of the knee is more common among the amputees than the non-amputees. Osteopenia of the neck of the femur on the amputated side is also a frequent secondary impairment among the amputees. The reason is believed to be inactivity and low bearing efforts of the hip. This is a serious threat to the amputee in case of falling while walking or transferring. The most frequent walking aid of the lower-limb amputees are crutches which cause high pressure on the palm and consequently on the carpal tunnel region. The incidence of the carpal tunnel syndrome among the users of crutches is higher than in non-users. To relieve the symptoms of the carpal tunnel syndrome, auxiliary crutches are to be combined with the forearm ones. Each secondary impairment leads to marked disability and handicap of the amputee. Consequently, functional outcome of the rehabilitation is worse than expected. Members of a rehabilitation team should be able to recognize, prevent and treat all secondary impairments of the amputees. | |
