Abstract     Doctors working in a prehospital setting are often facing situations where severe pain has to be dealt with immediately before hospital is reached. In cases like these we use peripherally acting analgesics (NSAIDs and paracetamol), peripherally acting analgesics with a spasmolytic effect (metamisol), spasmolytics (buscopolaminum, trospium) and centrally acting analgesics (opiates and ketamine in lower doses). In Prehospital Units situation is satisfactory, whereas no such thing could be said for 1 A and 1 B Units, where doctors seldom have to deal with emergency situations. They do not have as many suitable analgesics and they seldom give them intravenously because of the difficulties with establishing an intravenous line. In Slovenia we still don't have an unified teaching programme for emergency medicine. Licences to practise emergency medicine are still not granted although the statute has been accepted a few years ago. The use of drugs for emergency cases is not unified. The choice of drugs to be used by doctors working in a prehospital setting is in the hands of our Heads of Departments and their personal judgement about importance and usefulness of certain substancies.
