Abstract | Background: The standard method for detecting anti-HLA antibodies and determining their specificities is the complement dependent cytotoxicity assay (CDC). This technique is complex and time consuming. These are the reasons why more rapid, easier and more reliable techniques have been developed for the detection and characterization of anti-HLA antibodies, like the enzyme-linked immunosorbent assay (ELISA) and the flow citometry (FC). Goal: The goal of this study was to compare all three techniques: the standard CDC, ELISA and FC, in determining the presency and specificity of anti-HLA antibodies in patients awaiting cadaveric kidney transplantation. Methods: For that purpose 36 pre-transplant serum samples of selected patients were chosen. Taking into consideration the differences between the three techniques we then compared the panel teactivities (PRA) and the antibody specificities as assigned by the CDC, the ELISA and the flow cytometry. The results where then stattistically evaluated by calculating the correlation coeficients. Results: The correlation between the CDC and the ELISA was found to be the strongest (r* = 0,62), followed by the one between the CDC and the flow citometry (r* = 0,50) and the one between the ELISA and the flow citometry (r* = 0,58) (p < 0,05). Conclusions: In conclusion, we showed that all three techniques are correlated in the determination of PRA and comparable in specificities assignments. Taking into consideration the extensive polymorphism and the existing cross-reactivity within the HLA system, as well as the precise patient's allosensibilisation history, we strongly believe that all three techniques complement each other and represent the best possible solution evaluating the patient's alloimmune status. | |
