Abstrakt     Backaround: There are many operative methods to stabilize type B lateral malleolus fracture. In most European countries and in many Slovenian hospitals osteosynthesis with neutralisation plate is predominant method. In our hospital, on the contrary, minimal osteosynthesis in two different forms is usually performed for such fractures. The purpose of this study is to eveluate clinical results of minimal osteosynthesis in type B lateral malleolar fractures. Methods: In a retrospective study we invited all patients that had their malleolar fracture operated on for a late control at least 3 years after accident and surgery. 242 patients (73 %) responded to our invitation. Out of all we selected just type B fractures according to Weber's classification. 179 patients had a type B fracture. In 160 of those fractures we performed minimal osteosynthesis, in 17 cases we stabilised lateral malleolus with a plate in two cases just with cerclage wire. Minimal osteosynthesis was performed in two different forms. In 75 cases we used two or more lag screws and in 85 cases we stabilised fibula with two or more intramedullary K - wires and tension band wire. On a late controll we tried to find out how succesful is minimal osteosinthesis in type B fractures in general and for each method alone. We used Weber's scoring system evaluating functional, rentgenological and subjective late result. Results: 87.5 % of patients with B fracture, that were operated on lateral malleolus with methods of minimal osteosynthesis, had a good end result. In complex fractures of fibula (3 or more fragments), method of lag screws was more succesfull than method of K-wires. In individual analysis of patients with poor end result, we try to propose the most rationale operative method for diferent subtypes of B malleolar fractures. (Abstract truncated at 2000 characters).
