Abstrakt | The article describes a patient with clinical features of serotonine syndrome. A 35-yr-old woman was admitted following deliberate intoxication with tramadole, moclobemide (a monoaminooksigenase inhibitor), paroxetine (a selective serotonine reuptake inhibitor) and benzodiazepines. Upon admission she was unconscious, hyperthermic, diaphoretic with pronounced muscle rigidity. Toxicological analyses revealed the presenee of tramadol moclobemide and also paroxiten and zolpidem in smaller concentrations. After exclusion of other possible causes; serotonine syndrome was considered. Treatment was symptomatic with mechanical ventilation, sedation and neuromuscular block. The patient recovered spontaneously 24 hr later. Serotonine syndrome is characterized by varying degrees of cognitive, autonomic, and neuromuscular dysfunction and can only be triggered by drug therapy that increases central nervous system serotonin neurotransmission. | |
