Abstrakt     Background: Cephoperazone, a new, third generation cephalosporin, is a bactericidal antibiotic, resistance to which has increased only a little, despite its relatively wide use. In regular surgical practice its transferability, effectiveness and sideeffects have been studied in patients with various locations and clinical forms of infection. Methods: Following appropriate surgical treatment of the infection, 126 patients with additional complicating factors that aggravated the infection (tissue and organ ischemia and impaired immune defence) were given parenteral treatment with cephoperazone. Before, during and after the therapy specimen were removed for bacteriological isolation. The success of the treatment was evaluated as the decrease in general and local signs of infection and regarding the occurrence of harmful side-effects. Results: The therapy lasted on average 5.8 days (from 3 to 16). In 21 patients an antianaerobic antibiotic had to be added. In 91 patients (72.2 per cent) the results were very good, in 31 (14.6 per cent) good, but in 4 (3.3 per cent) poor. In 12 patients side-effects were found: mild in 5, intermediate in 5 and severe in 2 cases. Conclusions: The use of cephoperazone combined with surgical treatment of the infection was very successful in three quarters of the cases, being especially effective in infections of the bile duct, gall-bladder, intestines and the abdominal cavity in general, whether in therapeutic or prophylactic indications.
