Abstrakt     The rate of thrombolysis markedly decreases after a thrombosed vessel is partly reanalyzed and the remaining clot poses a serious risk for rethrombosis. We studied in vitro how thrombolysis depended on penetration of plasma containing thrombolytic agents - 0.2 microg/ml recombinant tissue-type plasminogen activator rt-PA or 250 IU/ml streptokinase (SK) - and the magnetic resonance contrast agent Gd-DTPA (1 mmol/) into non-occlusive retracted blood clots under conditions of either fast or slow tangential flow. Fully retracted whole blood clots were glued into plastic tubes. After that, they were pierced lengthways and connected to a perfusion system. The initial flow of plasma through the artificial perfusion channel was either fast (130.2±100.1 cm s-l, est. shear rate of 8.7x103s-1), or slow (7.9±2.4 cm s-1, est. shear rate of 0.5 x103s-1). Serial spin-echo MRI was used for measuring penetration of plasma and the remaining clot size on cross section. Penetration of Gd-DTPA-labeled plasma into clots was faster with rapid than with slow tangential flow: after 10 min of exposure to rapid flow, GdDTPA-containing areas were found inside retracted clots at a distance of 1.65 (1.06-3.6) mm, but at only 0.71 (0.3-1.35) mm from the channel border in clots exposed to slow flow (p<0.05). The remaining undissolved parts of cross-sections of clots exposed to rapid flow after 60 min were 0.63 (0.28-0.73) for rt-PA and 0.95 (0.89-0.99) for streptokinase (p=0.009). The corresponding values for slow tangential flow were 0.97 (0.96-0.98) for rt-PA and 0.99 (0.97-1.0) for streptokinase (p=NS). Thrombolysis with rt-PA was faster under conditions of rapid flow than under conditions of slow flow, significant differences were seen after 20 minutes and persisted throughout the time of observation.
