Abstrakt     Biliary tract infections are caused by Enterobacteriaceae, Gram positive cocci, Pseudomonas aeruginosa, and anaerobes. The role of enterococci is not known. Many infections are polymicrobial. The choice of antibiotics depends upon their in vitro activity and the concentration at the site of infections. Biliary obstruction may diminish the concentration of the antibiotics in bile. Severity of the disease and antibiotic resistance have to be taken into account in planning of the antibiotic treatment as well. Penicillins with beta-lactamase inhibitors, cefalosporines, quinolones, aminoglycosides, metronidazole and clindamycin are most commonly used. Antibiotic prophylaxis before biliary surgical and endoscopic procedures is recommended in patients at risk.
