Abstrakt     BACKGROUND The evaluation of the patients with obstructive jaundice is a common clinical problem. There is no general consensus on the best approach to diagnosing obstructive jaundice. In this study we show our first experiences with EUS as the first endoscopic procedure in patients with suspected obstructive jaundice of unclear origin. METHODS A prospective study included 103 patients (44 men, 59 women; mean age 66.2 years, range 20-94) with obstructive jaundice suspected on clinical and biochemical data who were referred for diagnostic evaluation between December 2002 and June 2003. EUS was performed in all jaundiced patients in whome the cause of obstruction was not determined using transabdominal US. All EUS findings were confirmed by ERCP. RESULTS On EUS 9 of 27 patients (33%) had benign papillary stenosis as the cause of obstructive jaundice, 8 patients (30%) had choledocholithiasis, 4 patients (15%) had periampullary carcinoma, 3 patients had chronic pancreatitis (11%) and in 3 patients (11%) EUS could not identify the exact cause of the obstruction. ERCP changed the diagnosis in 3 patients; two periampullary carcinoma were detected among patients diagnosed with EUS as the benign papillary stenosis, and 1 carcinoma was diagnosed in a case where EUS could not identify the exact cause of the obstruction. CONCLUSIONS Improvement in the EUS/EUS-fine-needle aspiration technique in the near future will further more establish it's role as the first endoscopic procedure in patients with suspected obstructive jaundice due to possible pancreatobiliary neoplasm or of unclear origin.
