Abstrakt     Strabismus is a pathological deviation of one eye in relation to the other eye. Congenital convergent paralytical strabismus has strong consequences for child's eyesight and aesthetic apearance. Since 1985 to 1992 early diagnosis of congenital paralytical strabismus on twenty children in their first year of age was recognized and retrospective study of the operations was made. One of the cases will be presented in this article since we continued the therapy with the surgery and orthoptic pleoptic training up to the child's 13th year to 2003 446 patients were operated for strabismus, out of which 107 were adults. In the retrospective study two cases of operation for strabismus on adult patients will be presented in greater detail. Therapy consisted of preoperative, operative and postoperative orthoptic-pleoptic training. For all patients the skiascopy was performed, eyeglasses or prism were prescribed and oclusion and orthoptic training were scheduled. When the abduction could not be restored, due to the paralysis of external eye muscles, a decision for the surgery was made. In the first case the operative therapy with retroposition of internal and resection of external rectus muscle of the right eye of a patient at age one is presented. In patient's second year retroposition of internal rectus muscle of the left eye was performed and at age seven, due to the vertical deviation, retroposition of the upper and resection of the lower rectus muscle and retroposition of the outer rectus muscle of the left eye was performed. The therapy continued with orthoptic training up to patient's 13th year. In the second case of the divergent concomitant strabismus, retroposition of both external and resection of both internal rectus muscles were performed. In the third case of konvergent paralytical strabismus bilateral retroposition of the internal and resection of external rectus muscle of the right eye were carried out.
