Abstrakt     Background. Ultrasonic diagnostics is of great importance for the early discovery of changes in the blood vessels and for preventing the consequences of cerebrovascular illness. In the past six years, the transcranial Doppler method, which provides information on blood circulation in the brain, has become particularly significant for evaluating brain hemodynamics. Methods. The authors disclose the possibilities of continuous monitoring of cerebral blood flow in the middle cerebral artery by transcranial Doppler sonography during carotid endarterectomy. They present the monitoring of changes in intracranial haemodynamics during individual phases of operative treatment of carotid stenosis on the neck. Thus any changes in cerebral blood flow can be followed, especially during individual critical phases of the operation such as clamping of branches of the carotid trunk and intraluminal shunt. Conclusions. Such continuous transcranial Doppler sonographic investigation during surgery gives important data on the successfulness of collateral cerebral circulation, and on the successfulness of the intraluminal shunt. It makes the early detection of air microembolism in arterial shunt possible and discloses the necessity of its immediate revision. The possibility of the early detection of threatening cerebral arterial spasm and immediate adequate drug treatment are also of extreme importance. The authors also present their first experiences in the introduction of transcranial Doppler sonographic monitoring of cerebral circulation during carotid endarterectomy.
