Abstrakt     Background. The aim of this study was to find out the influence of failed vascular procedure on conversion of below-knee to above-knee amputation level. Methods. It was a prospective and nonrandomized trial conducted during 1984 - 90. Only patients with critical chronical ischaemia where bellow-knee amputation was done were included. They were divided into two groups. ln the first group (112 patients, 88 men, 24 women, average 62.5 years, range 46-80 years; 28 patients with diabetes) there were patients where primary amputation was done. In the second group (108 patients, 87 men, 21 women, average 61 years, range 43 - 79 years; 22 patients with diabetes) there were patients where at least one vascular procedure was performed before amputation. All patients had before amputation a complete medical and vascular surgical evaluation (including angiography and Doppler studies). The rate of reamputation from bellow to above-knee, preoperative angiographic state, preoperative outspread of gangrene, preoperative clinical state and death rate were compared. Results. Sixteen patients (14 per cent ) were reamputated in the first group and 33 (30.5 per cent ) in the second (significant difference, x2 = 8.4, P less th. 0.01). Twentysix out of 33 reamputated patients in the second group had a history of two or more vascular procedures before the amputation was done. Conclusions. It is concluded that failed vascular procedure influences conversion of below-knee to above-knee amputation level in patients with a history of two or more vascular procedures only.
