Abstrakt     Background. The cause of anemia in chronic renal failure seems to be multifactorial The aim of our investigation was to verify the hypothesis that the causes of anemia are at a higher stem cell level Therefore, beside peripheral blood BFU E which require erythropoietin, we cultivated peripheral blood stem cells which do not require it - CFU GM and CFU GEMM. Patients and methods. Peripheral blood BFU E, CFU GM and CFU GEMM were studied in 65 uremic patients who were placed on maintenance hemodialysis three times weekly. All were anemic. In regard to creatinine concentration they were divided into Group 1 with creatinine concentration < 900 micromol/I (24 pts) and Group 2 with creatinine concentration > 900 micromol/I (41 pts). Hematopoietic colony forming cells from blood were assayed in a methylcellulose culture with growth factor addition. Results. The growth of colonies BFUE and CFU GM was significant inhibited in whole group of patients, growth of CFU GEMM was significantly inhibited in the Group 2. A positive correlation of the CFU GM and BFUE, CFU GEMM and BFUE and CFU GM and CFU GEMM colonies was found in the whole group of patients. There was a negative correlation between the growth of the CFU GEMM, CFU GM and BFU E colonies and the urea concentration in the entire group of patients. In Group 2 a negative correlation was found between the growth of the BFU E and CFU GM colonies and creatinine concentration. In the Group 2 a negative correlation between growth of the BFU E and CFU GM and urea concentration was found. Conclusions. According to our study, erythropoetin concentration, although normal, is still too low with regard to the grade of anemia. In patients the number of stem cell colonies was decreased significantly, correlating with the severity of uremia. This confirms the hypothesis that disturbed hematopoiesis in uremia is affected by additional factors and that their influence in uremia is permanent.
Izvleček     Izhodišča. Anemija pri kronični ledvični odpovedi najverjetneje nastane zaradi več dejavnikov. Namen naše raziskave je bil potrditi hipotezo, da so vzroki za nastanek anemije na višji ravni matičnih celic. Poleg BFU E, ki potrebujejo za rast eritropoetin, smo iz periferne krvi še kultivirali matične celice, ki eritropoetinov za rast ne potrebujejo: CFU GM in CFU GEMM. Bolniki in metode. BFUE, CFU GM in CFU GEMM iz periferne krvi smo raziskovali pri 65 bolnikih z uremijo, ki so se zdravili s hemodializo trikrat tedensko. Vsi bolniki so imeli anemijo. Da bi ugotovili vpliv uremije na rast kultur celic, smo bolnike razdelili v dve skupini: skupino bolnikov s kreatininom < 900 mikromol/L (24 bolnikov) in skupino s kreatininom > 900 mikromol/l (42 bolnikov). Hematopoetske matične celice smo kultivirali na metilcelulozi z dodatkom rastnih dejavnikov. Rezultati. Rast kolonij BFU E in CFU GM je bila statistično značilno inhibirana v celotni skupini bolnikov in rast CFU GEMM statistično značilno inhibirana pri bolnikih v skupini s keratininom > 900 mikromol/L. Pozitivna korelacija med rastjo kolonij CFU GM in BFU E, CFU GEMM in BFU E in CFU GM in CFU GEMM je bila ugotovljena v celotni skupini bolnikov, v isti skupini pa tudi negativna korelacija med rastjo kolonij CFU GEMM, CFU GM in BFU E in koncentracijo sečnine. V skupini 1 je bila ugotovljena negativna korelacija med rastjo kolonij BFU E in CFU GM in koncentracijo kreatinina, v skupini 2 pa negativna korelacija med rastjo kolonij BFU E in CFU GM ter koncentracijo sečnine. Zaključki. Glede na naše ugotovitve zaključujemo, da je koncentracija eritropoetina, čeprav v mejah normale, prenizka glede na stopnjo anemije. Pri bolnikih smo ugotovili signifikantno zmanjšano rast kolonij matičnih celic, ki so korelirale s stopnjo uremije. To potrjuje našo hipotezo, da je hematopoeza zavrta zaradi različnih dejavnikov.
