Abstrakt     Background. Despite well-defined operative technique and tactics, the surgeon still remains an influencing factor on results of operative treatment of colorectal cancer (CRC). Most of the influencing factors can be join together in department doctrine. Methods. The participating departments were categorized in small, medium and large, according to the volume of patinets included in the study. Statistic evaluation was done with Studen t test and survival with Kaplan Meier and log rank. Results. From February 1992 to January 1995, 438 patients were included in OIGIT study, of which 283 (64,6%) with colon cancer (CC) and 155 (35,4%) with rectal cancer (RC). The most common operations were sigma resection CC patients and iow anterior resection in RC patients. In medium departments sphincter sparing operation in aboral thirds was made in 29%, in small 52% in at large 49%. Intestinal border was 6,25cm in CC patient and 3,23cm in RC patients. Radial border was 3,1 mm in CC patients and 1,9mm in RC patients. Average number of operations done by the same surgeon was 5,6 in CRC patients, 4,1 in CC patients and 3,4 in RC patients. 28 surgeons(36%) were included in study with only one operation. Pathologists examine an average of 12,2 specimens, while 8 pathologists(24,2%) examine only one specimen. During hospital stay, the patients was transfused an average of 342m1 in CC group and 539m1 of blood in RC group. Complication rate was 16,6%, wound inflammation in 8,1%, anastomotic dehiscence 1,8%, peritonitis 0,5%. Non surgical complications occurred in 6% of all cases. The CC specimen included an average of 11,2 lymph nodes and in RC specimen 10,6 lymph nodes. Overall survival in CRC patients was 59,8%, in CC patients 63,4% and in RC patients was 52,9%. Conclusions. There are differences in treatment results regarding case volume between departments. (Abstract truncated at 2000 characters).
