Abstrakt     The role of transabdominal ultrasonography in the diagnostic assessment of choledocholithiasis or common bile duct stones is well known. All investigators have mainly considered two ultrasonographic criteria: the finding of stones in the common bile duct and dilatation of the bile duct. Common bile duct stones are often found in preoperative ultrasonography because of cholecystolithiasis. Dilatation of the common bile duct is only an indirect sign of stones and many studies have indicated low sensitivity and positive predictive value of this ultrasonographic finding. On the basis of these findings, ultrasonographic diagnostics of choledocholithiasis cannot be sufficient for the screening of the common bile duct stones, although it is useful with other diagnostic modalities in determining which patients need further invasive diagnostic or therapeutic procedures. In patients with unclear findings for common bile duct stones, more invasive procedures should be performed, endoscopic ultrasound or magnetic cholangiography in particular. EUS is recommended in diagnosing choledocholithiasis if transabdominal ultrasonography has failed and may serve to limit the more invasive and expensive endoscopic retrograde cholangiography to therapeutic use in choledocholithiasis. The aim of the present article is to present the ultrasonographic assessment of choledocholithiasis.
