Abstrakt     There is great variation in neurological deficits caused by stroke. Disorders of speech and language are among the most prominent of them. Aphasia is present in 33% of patients with acute stroke. The severity of aphasia may change significantly during the course of the disease. Clinical expression of aphasia may differ from patient to patient. Various disorders of speech and language correlate with damage to particular areas of the brain. Clinical picture of stroke patient includes additional deficits in higher mental function, such as emotional lability, behavior disturbances, depression, rage reactions, visual field defect and hearing disorders, which interfere with speech therapy. Expectations of stroke patient and his family may play important role in speech rehabilitation. Speech therapy can start in acute phase of the disease and continue later. The recovery from aphasia can take longer periods, even years. In Slovenija the majority of stroke patients with aphasia and other speech disorders are not included in speech rehabilitation programmes. The main reason for that is poor availability of speech therapists in those institutions, where stroke patients are treated. The methods of speech rehabilitation are specialized. Therefore it is advisable, that they are performed by a person who has been trained in this field. Hopefu.lly the number of speech therapists in our country will increase in near future. Rehabilitation of aphasic patient should start in acute phase of the disease at specialized institutions and continue in community based speech rehabilitation programmes.
