Abstrakt | Background: Peripheral neuropathy is a common neurological disorder. It may be caused by diseases of the nerves or be the result of systemic illnesses. Objectives: To study disorders of distinct nerve fibres which produce specific symptoms and signs. Methods and subjects: We studied 68 elderly patients, 46 women and 22 men. Detailed clinical neurologic examination and extensive neurophysiological evaluation of the sensory and motor peripheral nervous system was done. Findings: In all patients thermal specific and thermal pain sensitivity at the distal end of the lower extremities were found to be impaired. Quantitative and qualitative changes were evaluated. Conclusion: The mechanisms underlying pain are still undetermined. According to our findings the pain of peripheral nerve origin - as well as some forms of paraesthesias, dysaesthesias and other sensory phenomena - are related to alterations of the unmyelinated and small myelinated nerve fibre, rather than to the myelinated fibre loss. | |
