Abstrakt     The article deals with a 53-year old healthy and recreative sportsman with a severe chest pain appearing 30 minutes after a regular and planned sport activity (basketball). ECG demonstrated ST elevation in precordial leads (anterior AMI). He was treated by morphine, oxygen, nitroglycerine, aspirin, heparin and transiently by dopamine infusion due to a transient arterial hypotension. He was moved to the cardiac interventional laboratory, where coronary angiography revealed 1-vessel coronary artery disease with a thrombotic subtotal stenosis of LAD coronary artery. 15 minutes after infusion of eptifibatide intraluminal thrombotic defect almost disappeared. The residual steno sis was directly stented, followed by a combined platelet antiaggregation therapy with aspirin 100 mg and clopidogrel 75 mg per day as well as a lipid lowering statin. The contribution illustrates a rapid, adequate, modern and efficient therapeutic approach of AMI treatment in almost ideal circumstances with an excellent result for the sportsman. The author propose administration of a thrombolytic therapy (tissue plasminogen activator) especially in AMI cases striking as a »bolt from the blue« in a healthy person, if percutaneous coronary intervention is not available within 2 hours of a chest pain attack (»time is muscle«). The review of literature and causes of acute thrombotic coronary artery occlusion in a recreative sportsman are analysed.
