Abstrakt     Background: Objective evidence of cardiac dysfunction is necessary for the diagnosis of heart failure and echocardiography is the preferred method for its documentation. With echocardiography it is often hard to asses left ventricular (LV) systolic and even harder to asses left ventricular diastolic function. There are no generally accepted criteria for its diagnosis. Tissue Doppler imaging (TDl) enables simple and precise measurements of velocities of mitral annulus longitudinal motion that has been shown to correlate well with LV systolic and diastolic function. However, itis not known if TDI derived velocities correlate with severity of heartfailure. Methods: A total of 50 patients with chronic heart failure irrespective of its cause, rhythm or presence of significant mitral regurgitation were included in our study. Mitral annulus longitudinal velocities (early diastolic - Em and systolic - S„,) were measured using pulsed TDI at lateral and septal border. The severity of heart failure was assessed according to New York heart Association (NYhA) functional class and by measuring plasma levels of amino-tenninal pro-brain natriuretic peptide (NT-proBNP). Results: The Em velocity was 5.2 ± 1.4 cm/s and Sm velocity was 5.4 ± 1.5 cm/s. Plasma levels of NT-proBNP were elevated in all our patients (808.7 ± 906.6 pmol/1). A very good and significant (P < 0.001) correlation was found between E` velocity, plasma levels of NT-proBNP (r = -0.73) and functional class (r = -0.68). The correlation between Sm velocity, plasma levels of NT-proBNP (r = -0.36) and functional class (r = -0.38) was weaker, but significant (P < 0.01). Conclusion: TDI derived velocities of mitral annulus are correlated with the severity of heart failure, especially Em velocity. We can conclude that Em velocity is a simple and accurate echocardiographic marker of severity of heart failure.
