Abstrakt | Background The laparoscopic splenectomy is a method of choice for a safe removal of the spleen in patients with haemathologic diseases having usualy only a slight spleen enlargment if any. Patients and methods. In the period from January 1997 till June 2004 we have performed 37 laparoscopic splenectomies. The indication was in 32 patients a haemathologic disorder, 26 patients having idiopathic thrombocytopenic purpura, spherocytosis in four cases two hemolytic anemias and 5 patients were operated upon for benign splenic cysts. Patients are positioned on the operating table in a lateral decubitus "antitrendelenburg" position with the left flank lifted. In majority of cases only three trocars, an 'endostaple' and endobag have been used. Results. The mean operating time in our series was 125 minutes, ranging from 70 to 240 minutes. A conversion was necessary in three cases. Peroperative blood replacement was executed in five patients. The average hospital stay was 5 days.ln three patients we discovered and removed an accessory spleen. Two patients developed postoperatively a subphrenic abscess, which were drained percutaneously. | |
