Abstrakt | Background. Pseudocysts of the spleen are frequently posttraumatic. Patients after splenectomy or absence of splenic function are predisposed for sepsis and mortality in high rate. Pneumococcus, Haemophilus influenzae and meningococcus are most often isolated bacteria. Patient and methods. It is reported a case of 61 years old male, with CT and US diagnosed liquid collection in the spleen, 19 cm in diameter. Results. Posttraumatic pseudocyst filled with 1,6 liter of liquid blood was seen at surgery. Intraoperative bleeding was reason for splenectomy. The fourth postoperative day the patient become septic and bilateral bronchopneumonia was diagnosed. MRSA was isolated from the blood. On the tenth postoperative day the patient died. Conclusions. The trend for treatment of injuried spleen and spleen pseudocyst is towards conservative management and spleen preserving. In splenectomized patients the risk of severe septic complications is 200 times higher. Partial splenectomy and fenestration are the methods of choice. Patients need profilactic antibiotics and pneumococcal vaccine after surgery, but protection against septic complications, pneumonia and meningitis is not allways successful. | |
