Abstrakt     Topical steroids have been the conventional therapy for atopic dermatitis for 50 years. There are several concerns related to this therapy especially after a long term use. They act on broad spectrum of immune cells and also interfere with other skin cells like fibroblasts and endothelial cells. Tacrolimus ointment and pimecrolimus cream are new topically effective macrolides. Both were found safe and effective for the treatment of atopic dermatitis in children and adults. They inhibit inflammatory cytokines transcription in activated T-cells and other inflammatory cells through inhibition of calcineurin. Local risk related to corticosteroids like skin atrophy and others do not occur, nor do the systemic side effects. When used after the appearence of first signs and symptoms of the disease they have significant advantages over conventional therapy and spare the side effects of the steroids.
