Abstrakt     Primary goal for managing chronic hyperglycaemia is preventing chronic complications of diabetes. Drugs from several classes are available as monotherapy or in combinations. The main cause of hyperglycaemia in patients with diabetes type 2 is tissue insulin resistance. First line of treatment are drugs which improve insulin action in peripheral tissues. In this group are two classes of oral hypoglycaemic drugs - biguanides and thiazolidinediones. If the treatment is unsuccessful secretagouges of insulin such as sulphonylureas or glinides should be used. The last line of treatment is insulin. Pateints with type I diabetes must be treated with insulin from begining, because they suffer of absolute lack of insulin. For managing hyperglyeaemia we also use alpha-glucosidase inhibitors. One of them acarbose, significantly decreases cardiovascular risk.
