Abstrakt     Background. The size and morphology of secundum-type atrial septal defects (ASD II) are crucial for the selection of the treatment modality and have been studied in detail in children. Almost 80% of ASD II in children fulfill echocardiographic criteria for percutaneous closure. Defects grow over time. Changes of defect morphology with age and their impact on the treatment have yet not been determined. AIM. With the study we sought to determine echocardiographic characteristics of ASD II in adult patients, in particular its size and morphological variations. Results were compared with the data on children. Echocardiographic parameters predicting the treatment modality were also sought. Hypothesis. ASD II in adults are larger and morphologically more variable than in children, therefore, in adults fewer defects are suitable for percutaneous closure. Patients in percutaneous group have smaller defect size, smaller right atrium and right ventricle size and lower pulmonary arterial hypertension. Patients and methods. Sixty-four consecutive adult patients with isolated ASD II were included in the study (mean age 43.6-15.9 years). Clinical examination, ECG and echocardiogram were performed. Transthoracic echocardiogram was performed in all patients and transesophageal echocardiogram only in patients deemed suitable candidates for percutaneous closure after transthoracic examination (maximal defect size < 30 mm and at least 5 mm rims, except the superior anterior rim, which may be deficient or absent). Regarding echocardiographic parameters, patients were divided into percutaneous and non-percutaneous group. The data on the defect size and morphological characteristics were compared with the data of 141 children. (Abstract truncated at 2000 characters)
