Abstrakt     BACKGROUND: Use of diagnostic endoscopy is becoming widespread in medicine. Overuse might lead to excessive workload of staff and equipement, increased waiting times and alf the financial consequences. Increased waiting times are potentilafly harmfull for patients. AIM: Aim is to analyse indications and findings of upper gastrointestinal endoscopy (UGE) and colonoscopy, and to evaluate the appropriateness of referrals to UGE and colonoscopy in Slovenia. HIPOTHESIS: Recent studies have shown innappropriate use of gastrointestinal endoscopy according to accepted indications. We presumed that for Slovenia similar results could be achieved. METHODS: A prospective cross-sectional study in one week included 21 endoscopic centers through out the country, where a total of 788 UGEs and 296 colonoscopies had been performed. We examined appropriateness of UGEs and colonoscopies by using the guidelines of American Society for Gastrointestinal Endoscopy (ASGE) and the European Panel on Appropriatness of Gastrointestinal Endoscopy (EPAGE). Eventual differences between referrals of general practitioners and specialist doctors were also analyzed. Special evaluation of appropc-iteness of refferal was performed on newly discovered gastric and colorectal carcinomas. RESULTS: According to ASGE guidelines 40,5%, and to EPAGE guidelines 27,4% of UGEs were deemed inappropriate. The most common inappropriate indication was abdominal pain in patients aged 50 years or below without adequate medical treatment (43,3% according to ASGE). The rate of colonoscopies was 33,8% inappropriate according to ASGE and 22,6% according to EPAGE guidelines. Appropriateness of referrals between general practitioners and specialist doctors w-as similar. CONCLUSIONS: The present study showed that UGE is overused in Slovenia according to the ASGE and EPAGE criteria.
