Abstrakt     The paper presents the conventional therapy of atopic dermatitis and presents a new strategy in topical treatment. Topical steroids have been the conventional therapy for atopic dermatitis for the last few decades. There are several concerns related to this therapy, especially to its long-term use. Steroids have inhibitory effects on a broad spectrum of immune cells, fibroblasts and endothelial cells. Their adverse side effects are the reason for increasing public and professional prejudice against long-term steroid therapy. The discovery of calcium-inhibiting local macrolides, such as tacrolimus and pimecrolimus, is a huge step forward in the topical treatment of skin diseases. They inhibit calcineurin by inhibiting the secretion of inflammatory cytokines released from activated T-cells . They are safe and effective agents for treating atopic dermatitis in both adults and children. Unlike steroids, they produce neither local nor systemic adverse side effects. They have significant advantages over topical steroids when given to patients with the first signs of disease flare-up. Also, they help reduce the usage of topical steroids in patients with chronic disease.
