Abstrakt     The paper presents the case of resuscitation on 7 th february 2005- the day when Slovene emergency service faced collapse. The apparently simple case turned into an urgent case with the worst outcome. The 74 year old patient with poorly regulated diabetes and overweight was treated in emergency room for diabetic hyperosmolar syndrome. The patient showed disturbed consciousness, hypotension, tachicardia and tachipnoe. After initial therapy the consciousness improved and the anamnesis showed that earlier that day she had felt pressure in the chest. We performed the 12 channel ECG- no new lesion was shown. At the same time we electroconverted another female patient admitted to the out-patients with stenocardial pain at PSVT. The first patient already felt better and talked, but suddenly lost consciousness. The monitor showed bradiarythmia changing into asistolia. We immediately started resuscitation which turned to be unsuccessful. We made the diterential diagnosis of either death due to cardiac arrest or pulmonary embolia. The autopsy showed recent heart lesion and an old scar of the myocard as well as presence of fatal hyperglycemia.
