Abstrakt     Background: Normal or near-normal coronary arteries are found at angiography in 10 to 25% of women and in 6 to 10% of men presenting with acute coronary syndrome. The prognosis of these patients is not as good as assumed. Previous studies have shown that at least 20% of these patients have increased risk of later adverse cardiac events due to atherosclerosis-related endothelial dysfunction. Case report: A 55-year-old male was admitted at the Department of Cardiology due to unstable angina. Coronary angiography revealed 80% narrowing of the proximal left anterior descending artery. The narrowing became significantly less severe after intravascular application of nitroglycerine. After providing adequate therapy, cycloergometric testing confirmed myocardial ischemia at 5 MET. Considering the angiographic result, we decided for medicamentous treatment and meticulous preventative measures, despite frequent episodes of angina. Three months later and after cesation of current medications we performed control angiography, which showed regression of left anterior descending artery narrowing and absence of spontaneous coronary artery spasm. Conclusion: This case report shows that certain patients are suitable for medicamentous treatment preventative measures and can thus be spared an invasive treatment. The case also shows the importance of intracoronary nitroglycerine usage for diagnostics of coronary artery spasm.
