Abstrakt     Diuretics, such as furosemide, can be lifesaving for the patients with acute pulmonary edema. A long-term use can prove effective in achieving euvolemia and better quality of life in patients with sodium and fluid retention. However, diuretics can do harm. The most important adverse effects are acute and chronic RAAS activation, electrolyte disturbances associated with an increase in malignant ventricular arrhythmias and the risk of death, glucose intolerance, hyperuricemia, and the disturbed metabolism of calcium. Diuretics are required only in patients with clinical evidence of sodium and water retention. When left ventricular systolic dysfunetion is asymptomatic, diuretics can be harmful. Appropriate diuretic therapy is a key factor for safe utilization of the other obligatory background therapy of CHF.
