Abstrakt     Treatment of children with pulmonary disease (mainly asthma) is an unique challenge for physicians and pharmaceutical industry. It is important that prescribed medicine acts on the site of the disease, that is lung. The medicine express its action earlier and is safer and more efficient. The doses are lower compared to the systemic treatment. This is especialy emphasized in children. Inhalation is the easiest way of delivering the medicine into the lungs. The oldest devices used for inhalations are inhalators. They were the only device to be considered effective in near past. The latest research data confirmed that metered dose inhalers can be as effective or even more effective, as inhalators especially with chambers. Children have an unique problem of coordination between puff of the aerosol from MDI and its inpiration into the lungs. We can help the child with a spacer with a mask. The correct size and shape of the spacer enables the good deposition of the medicine into the lungs, even in small children. Considering basic principles of the spacers with MDI-s and inhalers makes treatment of acute and chronic pulmonary diseases very effective. Accesibility and effectivenes of these devices makes them superior ones the inhalators.
