Abstrakt     BACKGROUND. Gastric carcinoma is one of the most common causes of death in malignant diseases of gastrointestinal tract. Surgery, among other possibilities, is for now the only successful treatment. The aim of treatment is RO resection (no evidence of tumor) with appropriate lymphadenectomy. The TNM classification has become the principal method for assesing the prognosis for cancer patients and to provide a reliable means for reporting and comparing the results of treatments. The 4th edition of TNM classification from the year 1987 was based on the distance of the metastatic nodes from primary tumor. The new Sth edition of TNM classification from year 1997 is based on the number of positive lymph nodes metastasis. AIM AND HYPOTHESIS. Aim of the study was to evaluate, if new TNM classification gives the same prognostic information as the previous classification. Which of the two classifications is better is still under discussion so I tested the next hypothesis: The Sth edition of TNM classification for gastric cancer is better for assesing the prognosis of gastric cancer patients than the 4th edition of classification. The number of positive lymph nodes has a higher prognostic value than the distance of the metastatic nodes from primary tumor. METHODS. The study was based on patients who underwent RO resection with appropriate lymphadenectomy and with 15 or more dissected lymph nodes. With uniform clinico-pathological protocol the necessary data was gathered. Data was statistically analysed with the computer program SPSS v. 10.0. Survival time was computed with Kaplan-Meier method. The significance of prognostic factors was evaluated with univariant analyse and their statistic meaning was defined with logrank test.
