Abstrakt | Background. Anemia is a common comorbidity in patients with chronic heart failure. Compared to general population, the prevalence of anemia is much greater among heart failure patients and is increasing with the severity of the disease. Anemia therefore represents a strong independent prognostic factor of adverse outcome in these patients. Since chronic heart failure results from complex pathophsyiological processes and is interrelated with many other diseases, the developement of anemia in these patients may result from several potential mechanisms. The most important of these are renal insufficiency, age, female gender, diabetes mellitus and treatment with angiotensin converting enzyme inhibitors. Understanding the mechanisms responsible for the development of anemia and its treatment could offer a new approach into heart failure therapy. Aims. In spite of its impact on disease progress, the pathogenesis of anemia in heart failure remains undefined. Although recent data suggest that anemia in heart failure may be related to the activation of inflammatory response, the evidence is non-conclusive. The aim of our study was to better defme the patophysiologic background and prognostic factors in patients with heart failure and anemia, and to investigate the role of inflammatory response activation with this regard. Hypotheses. The hypotheses of our study were as follows: (1) Lower hemoglobin levels are associated with a higher mortality rate in patients with chronic heart failure. (2) Inflammatory response activation in patients with chronic heart failure and anemia is associated with more profound anemia and a higher mortality rate. (3) Renal insufficiency is associated with a higher mortality rate in patients with chronic heart failure and anemia. (4) Diabetes mellitus, older age, female gender and ACE inhibitor therapy are associated with a worse prognosis in patients with chronic heart failure and anemia.
