The heroin addiction itself has mental, physical and social consequences therefore diagnostics of comorbid mental disorders is more complicated. The aim of the research was to focus on problems on neuropsychopharmacotherapy of heroin addicts with comorbid disorder from different perspectives. In the first study, the presence of comorbid mental disorder of heroin addicts in the outpatient programme for addiction treatment was examined. In the third study the review of literature was perfomed, both results of our studies were added and the optimal pharmacotherapy of heroin addicts with comorbid mental disorders was discussed. The results of the first study revealed that addict's depressive disorder was the most frequent diagnose in our sample of heroin and the second one anxiety disorder. The second study showed the problem of unreported double frequencies. The analysis of psychopharmacotherapy of heroin addicts with comorbid mental disorder was done. Patients with withdrawal symptoms received antipsychotics and benzodiazepines. Patients with depressive symptoms who were not on methadone treatment received antidepressants and benzodiazepines. The last study is a good starting point for improvement of clinical practice as it points to the problematic part of diagnostie, therapy and cooperation between different services.
