Abstrakt     Introductory: 43% of the surface of Slovenia is karstic terrain. One of its distinguishing marks in underground caves, partly open to visitors, but mainly accessible only to experienced speleologists. Injuries among both are a permanent possibility. Now and then even a wild or domestic animal falls into a pit. Accidents occur also in urban underground systems (e.g. waterworks, sewers). The Speleological Rescue Service (SRS) is the only institution in Slovenia trained for rescuing activity in karstic and urban underground milieus. The SRS functions as a special branch within the Speleological Association of Slovenia. Method: The first part of the article presents the history of the SRS, its organization and technical capacity, based on the legal regulations of its activity; it further presents several typicalities of rescue procedures in caves, and a line is drawn between the SRS area of activity and other (rescue) teams' areas of activity. Thus the performers of emergency medicine (EM) are acquainted with the conditions under which the assistance of the SRS is required. The second part presents peculiarities of medical intervention during the rescuing process in caves. Suggestions are made for improvement. Rescuing intervention in caves is impeded by weak artificial light, so that the basic checkup of the injured person may be problematic. Moreover, the medical examination, the dressing of the wounds, the immobilization, and the intravenous access are hindered by the specific equipment and overalls of the injured speleologist. The temperature in the caves of Slovenia causes hypothermia of the immobile injured person. To avoid the danger, the SRS, besides well-known and generally applied measures, also makes use of a portable devicefor active warming (heat pack). (Abstract truncated at 2000 characters)
