Abstrakt     Insertion of a subclavian central venous catheter carries a small but well defined risk of iatrogenous ipsilateral pneumothorax. A small pneumothorax is usually well tolerated by most patients. Life threatening is a tension pneumothorax which usually develops in mechanically ventilated patients. Pneumonectomy is a surgical resection of one lung, causing a notable chest wall deformity. Ipsilateral shoulder usually lowers, the intercostal spaces and the space between the first rib and theclavicle narrows, the diaphragm elevates. This can cause severe problems when trying to insert acentral venous catheter into the subclavian vein - forcing us to try on the contralateral side? Probably not if a possibility of rapidly fatal iatrogenous pneumothorax is kept in mind. A case of contralateral iatrogenous pneumothorax following pneumonectomy is reported.
