Abstrakt | Heart failure is a widespread, though manageable disease; nevertheless several effective interventions are not being implemented in every-day practice. Heart failure clinics have emerged as an effective means of heart failure management and education providing a significant reduction in hospitalization rates and improvement of the quality of life. Design of heart failure clinics depends predominantly on local circumstances. Heart failure clinics can be either nurse-lead or physician-directed, and can provide elementary care (therapy optimization and patient follow-up) as well as more complex interventions, such as patient education, rehabilitation, social and psychological support, and even i.v. therapy, CRT, invasive diagnostic procedures or heart transplantation. | |
