Abstrakt     Acute heart failure can present itself as new onset in a patient without previously known cardiac dysfunction or as acute decompensation of chronic heart failure. Treatment consists of symptomatic measures and specific hemodynamic treatment with diuretics, fluids, vasodilators, and positive inotropic agents. Currently available positive inotropic agents, betal-adrenoceptor agonists and phosphodiesterase III inhibitors, increase intracellular calcium with subsequent increases in myocardial oxygen demand and arrhythmogenesis. Levosimendan is a new calcium sensitizer, which increases myocardial performance without substantial changes in myocardial oxygen consumption and neutral effects on heart rhythm. With stimulation of ATP-dependant potassium channels it produces vasodilation, thus decreasing preload and afterload. Via potassium channel activation levosimendan also exerts antiapoptotic effects.
