Abstrakt     Patient with rheumatoid arthritis (RA) have shorter life expectancy compared with general population. The main causes of death are atherosclerotic cardio vascular diseases. The aim of our study was to determine and than examine accelerated atherosclerosis in pre-menopausal RA patients (without diabetes and hytertension) compared with healthy controls and a group of pre-menopausal female patients with previous atherosclerotic event. Ure elucidated the importance of traditional risk factors, some non-traditional risk factors for atherosclerosis and the importance of antiphospholipid antibodies (aPL) in these patients coimected with citokins: interleukin (IL)-2, IL-6, tumor necrosis factor alpha (TNFalpha) and adhesion molecules (ICAM, VCAM). In our study 70 pre-menopausal female RA patients (mean age 42,04 years), 40 healthy female controls (mean age 41,65 years) and 21 pre-menopausal female patients with previous atherosclerotic event (mean age 46,85 years) were included. Using B-mode ultrasonography we compared intima-media thickness (IMT) and plaque occurrence in the carotid arteries in RA patients and controls. We investigated the corelations of these variables to laboratory risk factors for atherosclerosis, sensitive CRP (sCRP), levels of TNFalpha, IL-2, IL-6, ICAM and VCAM. Our special interest was to evaluate the importance of aPL in those patients. The IMT values were higher in RA patients compared with controls (mean 0,586 mm versus mean 0,479 mm, p<0,0001). RA patients had more plaques than controls (p=0,023). In RA patients we found statistical significant higher levels of sCRP, ICAM, VCAM, II, 2, I1,-6 and TNFalpha.
