Abstrakt     The diagnosis of tuberculosis in children (especially in young children) is more difficult as in adults because of the paucibacillary nature of childhood tuberculosis and the problems with the collection of specimens. This article presents: (i) a brief overview of the principles of the collection of the specimens and of the methods ` for laboratory diagnostics of tuberculosis, and (ii) a short analysis of bacteriologic methods used for the detection of tubercle bacilli in Slovenian children and teen-agers from 2002 to 2006. At least 3 early morning gastric aspirates, bronchoalveolar lavages or bronchial aspirates collected on consecutive days are routinely performed in young children who cannot cough. There are three bacteriologic methods for the detection of tubercle bacilli in the specimens: microscopic examination of acid fast bacilli (the quickest method, but with low sensitivity), nucleic acid amplification tests (better sensitivity and specificity, but inferior to culture) and isolation of tubercle bacilli by culture which is time consuming, but is still "gold standard" (the highest rate of positive results). From January 2002 to September 2006 13 children and teen-agers were registered with culture-positive tuberculosis in Slovenia. Seven of them (53.85%) were smear positive, and all had positive cultures fromgastric lavage, sputum, induced sputum or bronchoscopic specimens. In one case culture was positive from lymph node swab only. Genotyping of tubercle bacilli showed that three of our patients had non-clustered strains of Mycobacterium tubeculosis. That means that the index case for the infection of one child and two teen-agers hasn't been found yet.
